The NHTSA has not rated the 2023 CT5-V overall or in the frontal crash, but awarded it its maximum five stars for the side and rollover tests. The rest of the CT5 range is rated the full five stars overall and for the frontal crash and the structurally similar CT5-V should inherit this rating. The IIHS has not yet rated the CT5.

There's no lack of safety features, with the standard complement including eight airbags (dual front, front side, side curtain, and dual front knee), ABS brakes with EBD, advanced stability control systems inherited from the Corvette, and standard driver-assistance systems such as forward and reverse automatic emergency braking, blind-spot monitoring, rear cross-traffic alert, lane-keep assist, pedestrian detection, a following-distance indicator, automatic high beams, front and rear parking sensors, and GM's Teen Driver technology, ensuring your kids don't take the car for an unlawful joyride. Optional safety features include a surround-view camera, automated parking, and rear pedestrian alert. Cadillac's Super Cruise suite is available on the CT5 as part of two optional packages, incorporating hand-free driving on compatible freeways and on-demand lane changing.

The CT5-V was subject to three recalls in 2021 for a failing airbag light that may not indicate an airbag malfunction, curtain airbags that may not deploy properly, and running lights that may not deactivate. The latter two carried over to 2022, but so far, it's still recall-free for 2023. JD Power has not rated the 2023 CT5-V, but the newest available rating - for the normal 2022 CT5 range - was 81 out of 100 overall and 85 for Quality & Reliability. Cadillac also adds further peace of mind with a four-year/50,000-mile bumper-to-bumper limited warranty and six-year/70,000-mile powertrain warranty, matched with roadside assistance for the same duration. One scheduled maintenance visit in the first year is included too.
